How to register as a new patient online
Patients who have never registered with a GP in the UK before will need to bring a copy of their passport & include on the registration form the date that they came to live in the UK.
To become a patient at Old Town Surgery, complete the registration form available from reception. Alternatively, download and print the registration form through this link.
You will need to complete this by hand & ensure you sign the form and bring it to the surgery with 1 form of photographic ID (e.g. Passport, driving license etc) and 1 form of proof of address dated within the last 3 months (e.g. Utility bill, bank statement etc) to support your application.
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
All patients should complete the Registration, Ethnicity and practice patient questionnaire forms.

Registering online as a non-English speaker
You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website.
Go to the NHS entitlements migrant health page on the Government website.
Registering as a temporary patient
If you are away from home, you can register as a temporary patient with Old Town Surgery for up to three months. This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.
You can register as a temporary patient by completing our registration form available from reception.
Registering children
Children under 16 need to be registered by their parent or guardian. You can do this by completing the form available from reception and the below questionnaire on their behalf. You will need to provide a form of ID to prove that you’re their parent or guardian.
What to do if you’re registering on behalf of someone else
If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.
Change of address
If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.
You can contact us online to let us know about a change in your address.
Please also let us know if your telephone number or email address changes.
